USAP-Texas in the Dallas-Ft. Worth area is comprised of high-caliber anesthesia and pain practices that were part of Pinnacle Partners in Medicine. USAP-Texas is an anesthesia and pain medicine leader in North Texas.
USAP-Texas provides anesthesia for all types of routine surgical procedures in North Texas, as well as for specialty areas such as cardiac, obstetrical, pediatric (through our ARCH pediatric unit), trauma, and endoscopic anesthesia.
USAP-Texas is committed to providing comprehensive, quality, and cost effective anesthesia services. Our providers are Board Certified Anesthesia physicians and top-tier care team members. We encourage all of our members to participate in continuing medical education and we operate an ongoing quality assurance program in conjunction with our hospital partners. Our associates are also fully integrated into the hospital medical staffs by serving as members of various committees and on governing boards. We actively support clinical research as a way to improve pain medicine and provide our patients with new and improved treatments and devices.

L. Jerald Bays, MD
L. Jerald Bays, MD, is a member of the Clinical Governance Board. He is a board certified anesthesiologist and has been a member of Pinnacle since its inception in 1999. Dr. Bays earned his medical degree at the University of Oklahoma College of Medicine and completed his residency at the University of Texas at San Antonio.
David Blaylock, DO
David Blaylock, DO, is the Vice Chairman of the Clinical Governance Board, and was a member of the Pinnacle team that negotiated Pinnacle’s partnership with USAP. He joined the Fort Worth division of Pinnacle in 2008 and was elected to serve as Division Manager and Board Representative in 2011. He serves on multiple committees at THR Harris Methodist Hospital including Physician’s Health and Rehab, Surgery Operations, Anesthesia Division QRC and Total Joint. Dr. Blaylock has been active in starting an acute postsurgical pain program and setting up an Anesthesiology Simulation Lab.
Stephen Donica, MD
Stephen Donica, MD, is a member of the Clinical Governance Board. He has been in private practice in Dallas for 22 years and has served as President and Division Manager of the Frontier Division.
David Holder, MD
David Holder, MD, is a diplomate of the American Society of Anesthesiologists and has been in private practice for 29 years. He is the 2013-2014 chairman of the Department of Anesthesia at Texas Health Plano and is the standing Director of Obstetric Anesthesia at that facility. Dr. Holder is a longstanding member of the Trinity Division Executive committee. Dr. Holder completed his medical school and residency at the University of Texas Medical Branch in Galveston in 1985 where he served as Chief Resident in his final year.
Scott Holliday, DO, MBA
Scott Holliday, DO, MBA, is the Chairman of the Clinical Governance Board and a member of the USAP Board of Directors. Dr. Holliday was appointed by the Governor of Texas in 2008 to serve on the Texas Medical Board and is currently serving his second term. Dr. Holliday is a certified anesthesiologist by the American Board of Anesthesiology. He completed his medical education at Kansas City University of Medicine and Biosciences and his internship and residency at the University of Texas Health Science Center at San Antonio. Dr. Holliday also completed a master’s degree in Business Administration at the University of Texas at Dallas in 2011. He is a graduate of the University of Texas at Austin.
Steven Luke, MD
Steven Luke, MD, is a member of the Clinical Governance Board and has been a part of Pinnacle Anesthesia since its inception in 1999. He served on the Retirement and Managed Care Committees and has previously served as Treasurer. Dr. Luke received his medical degree from the University of Texas Southwestern Medical School. He completed his residency at Parkland Memorial Hospital and fellowship at Oxford University, England and is board certified in Anesthesiology.
Steven McCord, MD
Steven McCord, MD, has been a practicing anesthesiologist for over two decades and has been a member of Pinnacle since its inception in 1999. He is board certified in anesthesiology and is currently a member of the Medical Executive Committee at Las Colinas Medical Center. Previously, Dr. McCord served on Pinnacle’s Formation Committee and was the Medical Director of Anesthesiology at Las Colinas Medical Center. Dr. McCord graduated from Baylor College of Medicine. He completed his residency at Texas A&M Scott and White Memorial Hospital.
Scott Meril, MD
Scott Meril, MD, is a member of the Clinical Governance Board. He is the current Chief of Staff, the Patient Safety Champion and serves on the Board of Directors at Baylor Hospital in Garland. Dr. Meril serves on the Baylor Surgery Council, is a Baylor Physician Champion and a member of the Baylor Quality Alliance Anesthesia Subcommittee. He has served on the quality committee at both the hospital and group level for several years. Dr. Meril is a graduate of Texas A&M University. He obtained his medical degree from Southwestern Medical School. His internship and residency training was at Parkland Hospital in Dallas.
Jay Morris, MD
Jay Morris, MD, is a practicing board certified anesthesiologist and has been a member of Pinnacle since its inception in 1999. He is a member of the Clinical Governance Board and formerly served on Pinnacle’s QI committee. Previously he was Chairman of the Department of Anesthesia at Baylor Regional Medical at Grapevine. Dr. Morris obtained his undergraduate degree from the University of Texas at Austin, and completed his medical school and residency at the University of Texas Southwestern in Dallas.
Stuart Simon, MD
Stuart Simon, MD is a member of the Clinical Governance Board and is the Co-Divisional Manager of Physicians Anesthesia Practice. He serves as a member of the Pinnacle Human Resources Committee. Dr. Simon has been an Attending Anesthesiologist at Baylor University Medical Center for over 20 years and is a Clinical Assistant Professor at The University of Texas Southwestern Medical School. He is currently Chairman of the Department of Anesthesiology at North Central Surgical Center and has been the Medical Director of that institution since its inception. He is the Physician Champion for United Surgical Partners International for the development and implementation of the electronic health record. Dr. Simon received his medical degree from The University of Mississippi School of Medicine and completed his internship in Internal Medicine at Baylor College of Medicine in Houston. He did his residency in Anesthesiology at Parkland Memorial Hospital where he served as Chief Resident. He completed a fellowship in Cardiothoracic Anesthesia at The University of Texas Southwestern Medical Center. Dr. Simon is a Diplomate of the American Board of Anesthesiology
Chris Spikes, MD
Chris Spikes, MD, held and continues to hold numerous leadership positions including the Management Committee of the Arlington Division and his current position as the Chairman of the Department of Anesthesia at USMD Hospital Arlington. Dr. Spikes attended medical school at Baylor College of Medicine, earning his degree in 1994. He completed his internship at St. Joseph's Hospital, his residency in Anesthesiology at University College Health Science Center in 1997, and his residency in Pain Management-Anesthesiology at the University of Texas Southwestern Medical School in 1998.
Thomas Swygert, MD
Thomas H. Swygert, MD, is a member of the Clinical Governance Board and serves on the USAP Board of Directors. Dr. Swygert entered practice in 1984 and has served in a variety of leadership roles throughout his career. He is currently Chairman of the Quality Performance Committee of the Methodist Hospital for Surgery in Dallas. Dr. Swygert served as President of the Texas Society of Anesthesiologists from 2006-2007.
Tony Tisdale, DO
Tony Tisdale, DO, is a member of the Clinical Governance Board. Dr. Tisdale is a board certified anesthesiologist with 20 years of experience. He earned his medical degree from Oklahoma State University in 1991.

Paul Morrow, MD, is Division Manager for North Texas Anesthesia and past Chairman of Anesthesia at Texas Health Resources Dallas.
Dr. Morrow is a Dallas native, attending medical school and completing his anesthesia residency at UT Southwestern. He has been in practice since 1997.
Larry Scott, MD
Larry Scott, MD is a member of the Clinical Governance Board, the Operations Committee, and the Clinical Quality Committee. Dr. Scott is a certified anesthesiologist by the American Board of Anesthesiology. He attended college at DePauw University in Greencastle, Indiana, medical school at the Johns Hopkins University School of Medicine in Baltimore, Maryland and his residency at Massachusetts General Hospital in Boston, Massachusetts. Dr. Scott was on the faculty of Massachusetts General Hospital and Harvard Medical School prior to going into private practice in Dallas, Texas.
